Registered Nurse

Job Overview
We are seeking a skilled and compassionate Registered Nurse (RN) to provide clinical oversight and direct patient care in a home health setting. The RN will be responsible for patient assessments, care planning, and coordinating with healthcare providers to ensure optimal patient outcomes.

Key Responsibilities
Conduct comprehensive patient assessments
Develop and update individualized care plans
Administer medications and treatments as prescribed
Monitor patient health and progress
Educate patients and families about care plans and health management
Supervise and support HHAs and CNAs when necessary
Maintain accurate clinical documentation and reports

Qualifications
Active Registered Nurse (RN) license
Graduate of an accredited nursing program
Minimum 1–2 years of clinical experience preferred
Home health experience is a plus
Strong assessment and communication skills
